Lower leg reconstruction and Scalp reconstruction

Lower leg reconstruction
This woman had previously sustained a fracture of the tibia with loss of skin which was initially treated with a skin graft. The subsequent scar was unacceptable and soft tissue reconstruction was performed after the fracture was healed. An tissue expander was inserted beneath the skin of the posterior calf and progressively filled with saline over several weeks. The skin created by this process allowed the unsightly skin graft (and the expander) to be removed.

A range of tissue expanders

  Early r esult: (12 weeks) The contour of the skin is progressively settling and the scar slowly fading. The scar will continue to improve for several years.

Extensive Basal Cell Cancer of the scalp: Treatment and subsequent reconstruction utilizing tissue expansion

This patient had a large area of invasive basal cell carcinoma on the scalp which was initially treated by excision and closure of the wound with a skin graft. Two months later a tissue expander was inserted beneath the adjacent skin of the scalp and progressively expanded over several weeks. The skin created by this process allowed the temporary skin graft to be removed and a superior cosmetic result.

 

   
Initial problem: Ulcerated basal skin carcinoma Intermediate stage: A skin graft has been used to re-surface the wound after excision of the skin cancer. Whilst the skin cancer has been completely removed the cosmetic result ispoor at this stage.
   
A range of tissue expanders End of skin expansion process: A tissue expander has been placed under the scalp and progressively filled with fluid to create sufficient new skin so that the thin skin graft can be removed.
   
 
Final Result: after tissue expander has been removed and the newly created skin used to allow the skin graft to be removed for an optimal aesthetic reconstruction
